Partly-printed letter signed Gideon Welles, 8×10, Navy Department stationery, March 30, 1864.  Letter accepting the resignation of Henry T. Cook as a midshipman at Newport, in full:

Your resignation of your position as a Midshipman in the Navy, tendered in your letter of the 15th inst., is hereby accepted.

Docketed and signed on integral leaf by G.S. Blake, Commander of the U.S. Naval Academy.
